All incoming passengers who intend to take advantage of the Visa Waiver Program are required to complete the I-94W form online (see links above) before departure to the United States, but preferably at least 72 hours (3 days) in advance. The purpose of Form I-94/I-94W is to record the arrival and departure information of all visitors to the United States. Most visitors to the United States need to apply for a visa before entering the U.S., unless if the person is a citizen of one of the 27 visa waiver countries (France & Spain are included) and has a machine-readable passport. Citizens of visa waiver countries can enter the U.S. for up to 90-day visa-free.

Regardless of whether the visitor is entering the United States under the Visa Waiver Program, or if he/she has a visa, all visitors must complete either Form I-94 (white) or Form I-94W (green).

What is the difference between Form I-94 and Form I-94W?

  1. Form I-94 (white): It is issued to visitors who hold a valid U.S. visa, such as B-1, B-2, F-1, H-1B, etc.
  2. Form I-94W (green): Is issued to visitors who enter the U.S. under the Visa waiver program (http://travel.state.gov/visa/temp/info/info_1304.html . The "W" stands for "Waiver". This means that you waive your rights to review or appeal any decision regarding admissibility in the United States, and prevents you (in most cases) from extending your stay in the United States or changing immigration status. Normally this is not a problem, but it is important to be aware of in case on possibly changing status after you enter the United States.

Form I-94 and Form I-94W consist of two parts:
  1. Arrival Record: The top part of the form is kept by the immigration officer upon arrival
  2. Departure Record: The bottom part is your departure record which must be returned upon departure. The Visa Waiver Program requires all travelers to have a machine-readable passport in addition to other requirements described in the Visa Waiver Program Information Guide.

Airline, Carrier Code

Choose the carrier code for the airline on which you will be traveling to the United States, if your travel information is available. The carrier code is a two-character code assigned to each airline. The carrier code is optional to complete the application.

Airline, Flight Number

Enter the flight number for the airline on which you will be traveling to the United States, if your travel information is available. The flight number is optional to complete the application.

City Where You are Boarding

Enter the last city where you will board an aircraft prior to arriving in the United States on this trip. The city where the applicant will board is optional to complete the application.

 

Address While In the United States Information

Description

Address Line 1

Enter the number and street of the location where you will stay while in the United States. Do not include the city and state in this field. Address Line 1 is optional to complete the application.

  • If multiple locations are planned, enter the first address.

  • If the complete address is not known, enter the name of the hotel or location you will visit. You may update this information once confirmed.

  • If you are leaving the United States immediately and do not have an address, enter the words ”In Transit” and your final destination location.

Address Line 2

Enter apartment, suite, unit, floor, building, or other, if appropriate. Do not include the city and state in this field. Address line 2 is optional to complete the application.

City

Enter the city of the address where you will stay while in the United States. The city is optional to complete the application.

State

Enter the state of the address where you will stay while in the United States. The state is optional to complete the application.
